Job Summary and Qualifications
As a Registered Nurse (RN), you are key to helping us provide superior patient care. You will work to provide nursing care in accordance with the client’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.
We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.
What you will do in this role:
- Coordinate total client care by conducting comprehensive health and psychosocial evaluation, monitoring the client’s condition, promoting sound preventative practices, coordinating services and teaching and training activities
- Evaluate the effectiveness of nursing services to the client and family on an ongoing basis
- Perform admission, transfer, re-certification, resumption of care and discharge OASIS for our clients
- Actively participate in weekly case conferences, inservices, and any staff meetings
- Discuss with the supervisor problems concerning the clients, and discuss within the team to other case managers such as Physical Therapist, Licensed Practical Nurse, Occupational Therapist, Speech Therapist, and Medical Social Worker, regarding changes or concerns with client
- Participate in our client’s discharge planning process
- Maintain an on-going knowledge of current drug therapy
- Preparing plan of care for the LPN
- Submit clinical notes and other progress notes within 72 hours
Qualifications
- The ideal candidate will be a graduate from an accredited School of Nursing
- Hold a current RN license that is in good standing with the state board of nursing
- Working knowledge of Medicare regulations/guidelines is a plus
- The ideal candidate will be a skillful organizer, and with the principles of time management and be a great team player and leader
- Comfortable working with the following equipment: thermometer, B/P cuff and glucometer
- You must have a minimum of three years experience as an RN, home health experience is strongly preferred
- You must be able to provide two references
- You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation
- A current CPR certificate is required
- Reliable transportation, proof of valid car insurance, and a valid driver’s license
