PHYSICAL THERAPIST – (PT) PART-TIME/PRN
(HOME CARE AGENCY) RALEIGH, NC
Home Care Agency is looking for a Physical Therapist to join our home care agency.
As a Physical Therapist, you are responsible for initial assessment, reassessment, developing care plans and setting measurable goals for clients. You are charged with carrying out clients care as established in the Agency’s policies and procedures.
Responsibilities:
- Perform initial assessment and ongoing evaluation and treatment
- Develop and implement an individualized plan of care with measurable goals for patients
- Monitor patients’ response to treatment
- Conduct individual patient therapy regimens
- Supervise Physical Therapy Assistants
- Work to identify and solve clients’ concerns
- Complete appropriate documentation according to agency’s policies and procedures
- Teaches, supervises, and counsels the client and family in the physical therapy program.
- Investigates and resolves patient/family member concerns regarding patient care.
- Ensures the highest quality of care is provided to patients by reviewing documentations.
Requirements:
- Must be a graduate of an accredited physical therapy program
- Must be a licensed Physical Therapist in the State of North Carolina
- Possess Basic Life Support (BLS) certification
- Knowledge of state, federal and professional laws and regulations pertaining to physical therapy in home care
- Superior listening, processing, and problem-solving skills
- Excellent verbal and written communication skills

Physical Demands:
- Combination of sitting, standing, bending, reaching, stretching, stooping, walking, climbing stairs and moving intermittently during working hours.
