Job Summary:

The RNs provides skilled nursing care, and case management for assigned patients in the patients place of residence; coordinates and integrates services to ensure continuity of care; facilitates the achievement of quality, clinical, and cost outcomes; negotiates, procures and coordinates services and resources needed by the patient or family; creates opportunities and systems to enhance outcomes; and uses the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation through an inter-disciplinary team approach.

Essential Duties and Responsibilities:

  • Provides skilled nursing while working with an inter-disciplinary team and coordinating care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician. Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions and referrals.
  • Observes, assesses, evaluates, and plans interventions in relation to patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications. Assures continuity of care through effective communication, information exchange and case conferences with other disciplines, community health, support and social agencies.
  • Oversees care provided by Home Health Aides to assigned patients and evaluates compliance with plan of care.
  • Maintains accurate, current and complete patient records in accordance with Agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations.
  • Participates in team scheduling coordination and “on-call” rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required.
  • Participates in quality improvement activities and attends in-service training as assigned.

Qualifications:

Required- A current RN license in the State of Montana (BSN preferred), a valid driver’s license, appropriate auto insurance, and reliable transportation. 

Preferred- 2 years of recent acute care or home care experience including such clinical skills as venipuncture, catheterization, wound care and/or parenteral therapy.

Schedule:

Specific workdays and work hours are directed by supervisor. Typically, this is a full-time position with a business-hour schedule and a rotating schedule for on-call and holiday coverage.

On-call scheduling is assisted through a first-contact service that we contract with to minimize the disruption in our nurses lives; the service reduces the number of calls by approximately 50%.
