How You’ll Make a Difference:
As a Quality Coordinator/MA with Arkos Health, you will be responsible for coordinating, scheduling, and follow up on clinical care needs for patients as it relates to Healthcare Effectiveness Data and Information Set (HEDIS) and other quality STAR initiatives. The Quality Coordinator has performance driven duties that primarily include conducting in-home Point of Care testing for care gap closure, entering data into our data base to capture/reconcile all Point of Care tests being performed. They will contribute to the organization’s drive to improve the overall experience of patient care, including the quality of health, and work to reduce the cost of care across the population served.
Essential Duties and Responsibilities:
- Perform in-home visits as needed for gap closure under the essential duties
- Conduct point-of-care testing including diabetic A1c collection, blood pressure checks, colorectal screening kits, and other testing required by STARS/HEDIS measures
- Retrieve and Review data collected to confirm gaps in care are captured
- Coordinate and order preventative and diagnostic tests around gaps in care on behalf of the primary care physician utilizing standing orders/protocols required by the health plan
- Conducts outreach to specified patients on priority lists for any outstanding care gaps and schedule accordingly
- Educating patients on procedures that are being scheduled/performed and the reason for performing them, including results
- This is both an in-office/field position: candidate must be comfortable traveling up to 20% of the time locally
Qualifications/Skills:
- High school diploma or GED equivalent required
- Medical Assistant (MA) Certification and or Certified Nurse Assistant (CNA) Certification
- Current CPR required
- 2+ years’ experience in a healthcare environment and medical terminology
- Recent and related experience working with HEDIS STARS Program
- Proficiency in technical tools and computer systems not limited to Microsoft Word, Excel, and Power Point
- Experience in Electronic Health Records
- Familiarity of Medicare guidelines to perform patient screening’s
- Ability to perform multiply procedures (i.e.) ABI, DRE, Urine Dips if necessary and collect and process specimen samples
- Experience with verifying Eligibility and navigating through Insurance Portals
- Experience with CPT II codes,HCC’s and Medical Diagnosis
